Metformin Titration Guidelines
Introduction to Metformin Titration
- The American Diabetes Association recommends starting metformin at 500 mg once or twice daily with meals and increasing by 500 mg increments every 7 days until reaching the target dose, to minimize gastrointestinal side effects 1, 2
- The maximum dose is 2000 mg daily for most patients, with a target maintenance dose of typically 1000 mg twice daily 2
Standard Titration Protocol
- The standard titration protocol involves starting at 500 mg once or twice daily with meals and increasing by 500 mg increments every 7 days until reaching the target dose, with a maximum dose of 2000 mg daily 1, 2
- For extended-release formulation, start at 500 mg once daily with the evening meal and titrate upward by 500 mg every 7 days until maximum dose is reached, with a maximum dose of 2000 mg once daily 1, 2
Renal Function-Based Dose Adjustments
- For patients with eGFR ≥60 mL/min/1.73 m², no dose adjustment is needed, and eGFR should be monitored at least annually 1, 2
- For patients with eGFR 45-59 mL/min/1.73 m², initiate at half the standard dose and titrate to half the maximum recommended dose, with consideration of dose reduction in patients at high risk for lactic acidosis 1, 2
- For patients with eGFR 30-44 mL/min/1.73 m², reduce dose to 1000 mg daily, and for patients with eGFR <30 mL/min/1.73 m², discontinue metformin 1, 2
Special Populations
- For pediatric patients (≥10 years old), start at 500 mg once or twice daily and titrate up to maximum 2000 mg daily in divided doses as tolerated 4, 2, 3
- For patients with marked hyperglycemia, initiate insulin first if A1C ≥8.5% or glucose ≥250 mg/dL, and add metformin after metabolic stabilization and titrate as above 4, 3
Monitoring and Avoiding Pitfalls
- Monitor eGFR regularly, especially when <60 mL/min/1.73 m², and do not ignore renal function 1, 2
- Monitor fasting blood glucose regularly during titration to determine dose effectiveness, and consider adding a second agent if glycemic targets are not met after 3 months at maximum tolerated dose 3, 2
- For patients with established cardiovascular disease, CKD, or heart failure, consider adding an SGLT2 inhibitor or GLP-1 receptor agonist while continuing metformin 1, 2
