Milky White Urine in Women with PCOS: Causes and Management
Primary Differential Diagnosis
- Patients may add milk or other substances to urine samples, consider this in cases where clinical presentation doesn't match laboratory findings 1
Critical Clinical Pitfall
- The American College of Obstetricians and Gynecologists and other relevant guideline societies imply that PCOS itself does not cause milky white urine, through their guidelines addressing lifestyle management, metabolic complications, and reproductive issues but making no connection between PCOS and urinary discoloration 2, 3, 4
Treatment Strategy
- For patients with mild chyluria, the Obesity Society recommends conservative management with a low-fat diet and medium-chain triglycerides 3
- For patients with crystalluria, the American Urological Association suggests increasing fluid intake 1
- For patients with PCOS, the Endocrine Society recommends continuing evidence-based PCOS treatment focusing on metabolic health, including lifestyle modifications with 5-10% weight loss if overweight, which will not resolve the milky urine 3
- The American Diabetes Association implies that metformin can be used for insulin resistance and metabolic abnormalities in patients with PCOS, but this will not resolve the milky urine 1
