Community-Acquired Pneumonia Diagnosis
Indications for Chest Radiography
- The American College of Radiology recommends that chest radiography is not routinely performed in children over 3 months with uncomplicated pneumonia who do not require hospitalization and have a good general appearance 3
- The American Journal of Respiratory and Critical Care Medicine suggests that chest radiography is required in adults with respiratory symptoms (cough, sputum production, dyspnea) accompanied by fever and abnormal auscultatory findings to establish a diagnosis and differentiate it from acute bronchitis 4, 1
Utility of Chest Radiography
- The Clinical Infectious Diseases journal states that chest radiography confirms the presence of pulmonary infiltrate, a fundamental requirement for the diagnosis of pneumonia 1
- The Journal of the American College of Radiology notes that chest radiography documents the size, character, and distribution of parenchymal infiltrates 3
- The American Journal of Respiratory and Critical Care Medicine indicates that chest radiography identifies complications such as pleural effusion, lung abscess, or multilobar involvement 4, 1
Limitations of Chest Radiography
- The Journal of the American College of Radiology warns that chest radiography cannot reliably distinguish between viral and bacterial pneumonia, or between different bacterial pathogens 3, 5
Alternative or Complementary Studies
- The Journal of the American College of Radiology reports that thoracic ultrasonography has a sensitivity of 93-96% and specificity of 93-96% compared to radiography 3
- The American Journal of Respiratory and Critical Care Medicine states that computed tomography is not indicated as an initial study in uncomplicated pneumonia 3
Microbiological Studies
- The American Journal of Respiratory and Critical Care Medicine recommends blood cultures before initiating antibiotics (yield ~11%) in patients hospitalized with severe pneumonia 5
- The Clinical Infectious Diseases journal suggests antigen testing for Legionella in severe pneumonia or ICU admission 4, 1, 5
