Sertraline Discontinuation Syndrome Management

Introduction to Withdrawal Symptoms

  • The American Academy of Pediatrics recommends slow tapering of all SSRIs, including sertraline, to prevent withdrawal effects, with symptoms typically occurring within a few days of drug discontinuation and lasting a few weeks 3, 1

Tapering Approach

  • A reasonable approach for sertraline 150 mg involves reducing the dose by 25-50 mg decrements every 1-2 weeks, with a total taper duration of minimum 6-10 weeks for long-term therapy 1, 2
  • The American College of Physicians notes that older guidelines suggesting 10-14 day tapers are inadequate for long-term therapy, and recommends a more gradual tapering approach 4

Monitoring and Management

  • Patients should be monitored for withdrawal symptoms, including dizziness, sensory disturbances, or mood changes, and immediate reporting of severe or intolerable symptoms requiring dose adjustment is necessary 4, 1
  • The American College of Physicians recommends resuming the previously prescribed dose if intolerable symptoms occur, followed by decreasing the dose at a more gradual rate 4
