Caffeine Exposure During Pregnancy and Breastfeeding: Risks and Guidelines

Effects of Caffeine on Fetal Development

  • Caffeine freely crosses the placenta and blood-brain barrier due to its lipophilic nature, exposing the fetus to substantive amounts 1, 2
  • The European Food Safety Authority recommends strictly limiting caffeine intake to ≤200 mg per day during pregnancy (approximately 1-2 cups of coffee) 1, 2

Caffeine Intake and Breastfeeding

  • Caffeine can cause jitteriness, vomiting, bradycardia, and tachypnea in breastfed infants, with signs appearing at birth and lasting 1-7 days 3
  • Peak breast milk caffeine concentrations occur 30-60 minutes after maternal consumption, similar to alcohol 4, 5

Safe Caffeine Limits and Guidelines

  • Women with uncontrolled hypertension should avoid caffeine entirely until blood pressure is adequately controlled 2
  • The American Academy of Pediatrics recommends that breastfeeding mothers monitor infants for jitteriness, vomiting, or sleep disturbances and time caffeine consumption to minimize breast milk concentrations during feeding 3, 4, 5

Caffeine Content in Common Beverages

  • Coffee: 50-300 mg per 8 oz serving (depending on brewing method) 1, 2
  • Tea: 15-50 mg per 8 oz serving 1
  • Decaffeinated coffee/tea: ~10 mg or less per 8 oz 1
  • Energy drinks and soft drinks: variable content, must be counted toward daily total 1, 2
