Management of Iatrogenic Iron Deficiency in Hemochromatosis

Immediate Management Steps

  • Interrupt phlebotomy therapy immediately when ferritin falls below 500 mcg/L and continue monthly monitoring, as recommended by the European Association for the Study of the Liver (EASL) 1, 2
  • The EASL guidelines are explicit that therapy should be interrupted at this threshold to prevent iron deficiency 1
  • Do not resume phlebotomy until ferritin rises above target maintenance range, according to the American Association for the Study of Liver Diseases (AASLD) and EASL guidelines 2

Prevention: The Critical Monitoring Algorithm

  • Check hemoglobin before every phlebotomy session, and reduce phlebotomy frequency or volume if hemoglobin falls below 12 g/dL, as recommended by the British Society for Haematology 2
  • Measure serum ferritin monthly or after every 4th phlebotomy, and check it every 1-2 phlebotomy sessions when ferritin decreases below 200 mcg/L to avoid iron deficiency 2
  • Target ferritin levels during induction phase: 50 mcg/L (EASL), 50-100 mcg/L (AASLD), and 20-30 mcg/L (British Society for Haematology) 2
  • Target ferritin levels during maintenance phase: 50-100 mcg/L (EASL and AASLD), and <50 mcg/L (British Society for Haematology) 2
  • Consider dose reduction if ferritin falls below 1000 mcg/L at 2 consecutive visits, especially if dose >17.5 mg/kg/day, as recommended by the EASL 1
  • Interrupt therapy if ferritin falls below 500 mcg/L, as recommended by the EASL and AASLD 1, 2

Special Considerations

  • More relaxed ferritin targets (<200 mcg/L for women, <300 mcg/L for men) may be better tolerated during maintenance phase in elderly patients, though this is based on expert opinion rather than clinical studies, according to the Praxis Medical Insights 2

Long-Term Management After Recovery

  • Resume maintenance phlebotomy at reduced frequency (every 1-4 months) once ferritin recovers to target range, as recommended by the Praxis Medical Insights 3
  • Target ferritin 50-100 mcg/L and transferrin saturation <60% during maintenance phase, according to the Praxis Medical Insights 3
  • Monitor ferritin every 6 months during maintenance phase, and investigate unexpected changes in ferritin or transferrin saturation, as recommended by the Praxis Medical Insights 3
