Causas y Aplicaciones de la Elevación de Beta 2 Microglobulina

Causas Principales

Elevated Beta-2 Microglobulin: Clinical Significance

Primary Clinical Indications

  • Elevated beta-2 microglobulin (β2M) reflects tumor cell mass and is a standard measure of tumor burden in multiple myeloma, serving as a strong independent prognostic indicator for treatment-free interval, treatment response, and overall survival, according to the National Comprehensive Cancer Network 10, 11
  • The International Staging System (ISS) for multiple myeloma stratifies patients based on β2M levels: Stage I (<3.5 mg/L), Stage II (3.5-5.5 mg/L), and Stage III (≥5.5 mg/L), with Stage III associated with poorest outcomes, as stated by the International Staging System 12

Diagnostic Workup When β2M is Elevated

  • Initial laboratory assessment should include complete blood count, comprehensive metabolic panel including creatinine and calcium, and lactate dehydrogenase (LDH) to assess tumor burden, as recommended by the National Comprehensive Cancer Network 10, 11
  • If multiple myeloma is suspected, skeletal survey or advanced imaging (CT, MRI, or PET-CT) to identify lytic lesions, and FISH cytogenetics to identify high-risk abnormalities: del(17p), t(4;14), t(14;16), as suggested by the Annals of Oncology 12

Prognostic Stratification

  • The Revised International Staging System (R-ISS) incorporates ISS stage (based on β2M and albumin), high-risk cytogenetics by FISH, and elevated LDH (>upper limit of normal), as stated by the Annals of Oncology 12

Critical Clinical Pitfalls

  • In patients with creatinine >2 mg/dL or CrCl <40 mL/min, β2M may overestimate tumor burden, and consideration of calculating corrected β2M or using alternative prognostic markers is recommended, as noted by the Annals of Oncology 12
