Management of Perineural Cyst on Thoracic Spine
Assessment and Initial Management
- Initial evaluation should focus on determining if the cyst is causing spinal cord compression, nerve root impingement, CSF obstruction, visual disturbances, or hydrocephalus 1, 2, 3
- Visual disturbances should be evaluated if relevant to the location of the cyst 1
- Hydrocephalus is a potential complication that should be assessed 1, 2, 3
Management Algorithm Based on Symptomatology
For Asymptomatic or Mildly Symptomatic Cysts:
- Conservative management is strongly recommended for asymptomatic or mildly symptomatic cysts, especially considering the patient's advanced age 1, 2
- Patient counseling and reassurance about the benign nature of the finding is essential 1, 2
- A follow-up MRI scan after 1 year to demonstrate stability may be reasonable, but long-term imaging follow-up is not necessary for asymptomatic cysts 1, 2
Follow-up Recommendations
- For asymptomatic cysts, routine clinical follow-up without mandatory imaging is recommended 1, 2
- Consider repeat imaging only if there is clinical deterioration 1, 2
