Indications for ERCP in Biliary Pancreatitis

Primary Indications for ERCP in Biliary Pancreatitis

  • The American College of Radiology suggests that ERCP with sphincterotomy is strongly indicated for acute gallstone pancreatitis with cholangitis, as it significantly reduces mortality and both local and systemic complications (Grade 1B evidence) 1, 2, 3
  • The World Journal of Emergency Surgery indicates that ERCP is indicated for acute gallstone pancreatitis with common bile duct obstruction, as it has been associated with significant reduction in local complications (Grade 2B evidence) 1, 2, 3
  • For patients with severe gallstone pancreatitis without clinical improvement within 48 hours, urgent ERCP and sphincterotomy may reduce overall morbidity 4, 6
  • The Gut journal recommends that increasingly deranged liver function tests with signs of cholangitis require immediate therapeutic ERCP 4, 6

Therapeutic Benefits of ERCP in Biliary Pancreatitis

  • ERCP can achieve clearance of CBD stones with success rates of 80-95% 7, 8
  • The World Journal of Emergency Surgery states that ERCP can significantly reduce local complications in patients with biliary obstruction 1, 2, 3
  • ERCP can be definitive management in medically unfit patients who cannot undergo cholecystectomy 4, 6

Potential Complications and Considerations

  • ERCP carries a 4-5.2% risk of major complications, including pancreatitis, cholangitis, hemorrhage, and perforation 7, 8, 9
  • The mortality risk is approximately 0.4% following ERCP 7, 8
  • The risk of iatrogenic pancreatitis is up to 10% following sphincterotomy 7, 8
  • ERCP should always be performed under antibiotic cover to reduce infection risk 4, 6
