Management of Akathisia in Patients Taking Antipsychotics

Initial Assessment and Dose Optimization

  • Haloperidol carries a 20% risk of extrapyramidal effects, and efficacy shows diminishing returns above 10-15 mg daily, according to the American College of Clinical Pharmacology 1

Pharmacological Treatment Algorithm

  • Lorazepam 0.5-1 mg orally as needed is a reasonable choice for treating akathisia, particularly if subjective distress persists, as recommended by the British Medical Association 2

Monitoring Requirements

  • Both antipsychotics and many alternative treatments can prolong QTc interval, requiring cardiac monitoring in at-risk patients, as advised by the American Heart Association 1

Practical Treatment Sequence

  • Reducing the antipsychotic dose to 5-10 mg daily, if clinically feasible, is recommended by the American College of Clinical Pharmacology as a first step in managing akathisia 1
