Gabapentin Tapering Protocol

  • The American College of Cardiology is not applicable here, however, a tapering protocol for gabapentin recommends reducing the dose by 100mg every 2-3 days, with complete discontinuation achievable within one week, starting with a reduction from 300mg daily to 200mg daily on Day 1-2, then to 100mg daily on Day 3-5, and discontinuing completely on Day 6-7 1

Monitoring During Tapering

  • Monitoring during tapering should include assessing for sleep disturbances, as well as pain exacerbation, particularly if gabapentin was used for neuropathic pain, according to the Mayo Clinic Proceedings 2

Special Considerations for Inpatient Setting

  • For patients with severe comorbidities or the elderly, the guideline recommends considering an even more gradual taper with smaller decrements, such as 50mg reductions 1
