Topical Moderate Potency Steroids for Inflammatory Skin Conditions

Introduction to Moderate Potency Topical Steroids

  • The American Academy of Dermatology recommends moderate potency topical corticosteroids (class 3-5) for inflammatory skin conditions when treating areas not involving the face, intertriginous areas, or areas susceptible to steroid atrophy 1, 2

Clinical Applications

  • Moderate potency topical corticosteroids are indicated for plaque psoriasis (not involving intertriginous areas) 1
  • The American Academy of Dermatology suggests moderate potency topical corticosteroids for atopic dermatitis (moderate cases) 3
  • Moderate potency topical corticosteroids can be used for scalp psoriasis 1

Duration of Treatment

  • Moderate potency corticosteroids can be used for up to 4 weeks for plaque psoriasis 1
  • For maintenance therapy, intermittent use (2 times/week) of moderate potency corticosteroids can help reduce disease flares and relapse 3

Application Guidelines

  • The American Academy of Dermatology advises avoiding use of moderate potency topical corticosteroids on face, intertriginous areas, and areas susceptible to steroid atrophy 1, 2
  • For thicker, chronic plaques, higher potency corticosteroids may be required 1
  • For sensitive areas, lower potency corticosteroids are recommended 2

Potential Adverse Effects

  • The risk of systemic effects from moderate potency topical corticosteroids increases with higher potency 2

Special Considerations

  • Patient preference should be considered for adherence to topical corticosteroid treatment 2

Monitoring

  • Reassess after 2-4 weeks to determine if continued treatment with moderate potency topical corticosteroids is necessary 4, 5
  • The American Academy of Dermatology recommends choosing the appropriate potency of topical corticosteroid based on the severity of the condition, location of application, patient age, and duration of treatment needed 1, 2
