Ramipril Dosing Guidelines

Introduction to Ramipril Dosing

  • The maximum recommended dose of ramipril is 10 mg twice daily (20 mg total daily dose) for heart failure post-myocardial infarction, and 20 mg daily for hypertension, as recommended by the American Heart Association 1

Dosing by Indication

  • The European Society of Cardiology guidelines recommend a target maximum dose of 5 mg twice daily (10 mg total daily dose) for heart failure post-myocardial infarction, with a starting dose of 2.5 mg twice daily (or 1.25 mg twice daily if hypotensive) 1
  • The target dose of 5 mg twice daily was used in the AIRE trial and demonstrated mortality benefit in patients with heart failure post-myocardial infarction 2, 3, 4
  • For chronic heart failure, the European Society of Cardiology guidelines recommend a maintenance dose range of 2.5-5 mg twice daily, with a starting dose of 1.25-2.5 mg daily 2, 3, 4

Monitoring and Follow-up

  • After initial dosing, patients should be observed for at least 2 hours and until blood pressure stabilizes for an additional hour, and serum potassium and creatinine should be checked after 5-7 days of initiation or dose changes 2, 3, 4

Ramipril Dosing and Management

Introduction to Ramipril Therapy

  • The European Society of Cardiology recommends ACE inhibitors like ramipril as first-line therapy for hypertension, preferably in combination with a dihydropyridine calcium channel blocker or diuretic 5

Heart Failure Management

  • The American College of Cardiology and American Heart Association list ramipril with an initial dose of 1.25-2.5 mg once daily and a maximum dose of 10 mg once daily for heart failure 6
  • The target dose of 5 mg twice daily was used in the AIRE trial and demonstrated a 27% reduction in relative mortality risk 7

Long-Term Management and Safety

  • Persistent cough should be tolerated if not severe, given the long-term mortality benefits 6
  • Ramipril should be maintained lifelong, even beyond age 85, if well tolerated 5
  • The European Society of Cardiology recommends avoiding combination of ramipril with NSAIDs, COX-2 inhibitors, potassium supplements, potassium salt substitutes, or potassium-sparing diuretics unless absolutely necessary 5
