Treatment of Skin Lesions with 5% 5-Fluorouracil

Standard Treatment Regimens by Indication

  • The British Association of Dermatologists recommends applying 5% 5-fluorouracil cream once or twice daily for 3-4 weeks, repeated if required, for squamous cell carcinoma in situ (Bowen's disease) 1, 2
  • Applying 5% 5-fluorouracil cream once daily for 1 week, then twice daily for 3 weeks, achieved an 83% complete response at 3 months in a large European multicentre RCT for squamous cell carcinoma in situ (Bowen's disease) 1, 2
  • Treatment may be repeated at 6 weeks if clinically indicated for squamous cell carcinoma in situ (Bowen's disease) 3
  • Twice daily application of 5% 5-fluorouracil cream for up to 9 weeks resulted in only an 8% recurrence rate at long-term follow-up for squamous cell carcinoma in situ (Bowen's disease) 3

Key Clinical Considerations

  • Variable treatment periods, typically 3-4 weeks, have been used in studies for squamous cell carcinoma in situ (Bowen's disease) 2, 3
  • Twice daily application of 5% 5-fluorouracil cream for 4-5 weeks is recommended for erythroplasia of Queyrat (penile lesions), though inflammation frequently limits this regimen 3, 4
  • Less aggressive regimens, such as twice daily application on 1 day per week for 12 weeks, provide disease control but not cure for squamous cell carcinoma in situ (Bowen's disease) 3

Site-Specific Considerations

  • Lower leg lesions in elderly patients may require observation rather than treatment due to poor healing potential 1, 2
  • Facial lesions typically use the standard 3-4 week regimen of 5% 5-fluorouracil cream 1, 2
  • Digital and nail bed lesions are generally fair to poor choices for 5-FU treatment 1, 2

Common Pitfalls to Avoid

  • Concentrations lower than 5% are less effective and should not be used 3, 4

Evidence Quality Note

  • The British Association of Dermatologists guidelines provide a Strength of Recommendation B with Level of Evidence 1+ for 5-FU in treating squamous cell carcinoma in situ, with an 83% initial response rate 1, 2
