Needle Decompression Recommendations for Pneumothorax

Adult Needle Decompression Sites

  • For left-sided tension pneumothorax, the 2nd intercostal space along the midclavicular line is the safest option due to lower risk of cardiac injury 1, 2
  • For right-sided tension pneumothorax, either the 2nd intercostal space midclavicular line or the 5th intercostal space midaxillary line can be used effectively 1, 2
  • Needle length should be at least 7 cm to ensure adequate penetration into the pleural cavity 1, 2
  • Failure rates are higher in patients with increased BMI 1
  • Risk of cardiac injury is higher with left-sided 5th intercostal space anterior axillary line approach 1, 2

Children Needle Decompression Sites

  • For pediatric patients, the 2nd intercostal space midclavicular line remains the recommended site 3
  • Needle length should be appropriate to the child's size and body habitus 3
  • The same anatomical landmarks apply, but special attention must be paid to the smaller anatomical structures in children 3

Technical Considerations

  • Insert needle perpendicular to chest wall at the superior border of the rib to avoid neurovascular bundle 4, 5
  • Confirm entry into pleural space by observing air escape or bubbling 4, 5
  • Leave the cannula in place until a functioning intercostal tube can be positioned 4, 5

Clinical Indicators for Tension Pneumothorax

  • History of chest injury 3
  • Progressive respiratory distress with labored breathing 3, 4
  • Absent or diminished breath sounds on the affected side 3, 4
  • Elevated chest wall, subcutaneous emphysema, and jugular vein distension 3
  • Hypotension, tachycardia, and shock 3, 4
