Esmolol Administration Guidelines

Introduction to Esmolol Administration

  • The American Heart Association recommends administering an IV loading dose of 500 mcg/kg over 1 minute, followed by an initial maintenance infusion of 50 mcg/kg/min, which can be titrated up to a maximum of 300 mcg/kg/min based on heart rate response 1, 2

Preparation and Administration

  • Calculate the loading dose based on actual body weight: 500 mcg/kg (0.5 mg/kg) 1
  • Administer the loading dose over 1 minute via IV push or rapid infusion 1, 3
  • Begin with 50 mcg/kg/min (0.05 mg/kg/min) immediately after loading dose 1
  • If response is inadequate, administer a second loading bolus of 500 mcg/kg over 1 minute and increase maintenance infusion to 100 mcg/kg/min (0.1 mg/kg/min) 1, 3

Monitoring and Contraindications

  • Continuous cardiac monitoring is essential 4, 5
  • Monitor heart rate response to ensure adequate rate control 6
  • Target heart rate depends on clinical indication: for strict rate control, resting heart rate <80 bpm, and for lenient rate control, resting heart rate <110 bpm 6
  • Use with caution or avoid in patients with asthma or obstructive airway disease, decompensated heart failure, pre-excited atrial fibrillation or flutter, cardiogenic shock, or severe bradycardia or heart block 1, 3, 7
  • Bradycardia is a common adverse effect 1, 3
