Insulin Glargine U-300 Dose Adjustment Guidelines

Standard Titration Algorithm

  • The American Diabetes Association recommends increasing Toujeo by 2 units every 3 days when fasting glucose is 140-179 mg/dL, or by 4 units every 3 days when fasting glucose is ≥180 mg/dL, until reaching a target fasting glucose of 80-130 mg/dL 1
  • The target fasting plasma glucose is 80-130 mg/dL, according to the American Diabetes Association 1
  • If hypoglycemia occurs without a clear cause, the American Diabetes Association recommends immediately reducing the Toujeo dose by 10-20% 1

Critical Threshold: When to Stop Escalating Toujeo

  • The American Diabetes Association recommends stopping the increase of basal insulin and instead adding prandial insulin or a GLP-1 receptor agonist when Toujeo exceeds 0.5 units/kg/day and approaches 1.0 units/kg/day 1
  • Clinical signals indicating the threshold has been reached include basal insulin dose >0.5 units/kg/day, bedtime-to-morning glucose differential ≥50 mg/dL, episodes of hypoglycemia, and high glucose variability throughout the day 1

Monitoring Requirements

  • Daily fasting blood glucose monitoring is essential during titration, according to the American Diabetes Association 1
  • Reassess every 3 days during active titration to adjust the dose, as recommended by the American Diabetes Association 1

Common Pitfalls to Avoid

  • The American Diabetes Association recommends never continuing to escalate Toujeo beyond 0.5-1.0 units/kg/day without addressing postprandial hyperglycemia, as this leads to suboptimal control and increased hypoglycemia risk 1
  • Never reduce the dose based on a single low reading—look for patterns over 2-3 days before making adjustments, according to the American Diabetes Association 1

Foundation Therapy Maintenance

  • The American Diabetes Association recommends continuing metformin (unless contraindicated) throughout Toujeo titration, as the combination provides superior glycemic control with reduced insulin requirements and less weight gain 1
