Treatment of Strep Throat

First-Line Treatment Recommendations

  • The American Academy of Pediatrics recommends penicillin or amoxicillin as first-line treatment for Group A Streptococcal (GAS) pharyngitis due to proven efficacy, safety, narrow spectrum, and low cost 1
  • The standard treatment duration is 10 days for most oral antibiotics, including penicillin and amoxicillin 1
  • Once-daily amoxicillin (50 mg/kg/day, maximum 1000 mg) for 10 days has been shown to be effective with improved adherence due to convenient dosing 1
  • There has never been a documented case of penicillin-resistant Group A Streptococcus anywhere in the world 1

Role of Augmentin in Strep Throat Treatment

  • The Infectious Diseases Society of America advises against using broad-spectrum antibiotics like Augmentin as first-line therapy, as it increases costs and promotes antimicrobial resistance without providing additional clinical benefit 1

Appropriate Use of Augmentin for Strep Throat

  • Augmentin may be considered for treating chronic GAS carriers, with a recommended dose of 40 mg amoxicillin per kg per day in three doses, maximum = 2,000 mg amoxicillin per day, for 10 days 2, 3
  • The American Family Physician recommends Augmentin (40 mg amoxicillin/kg/day in three doses) for 10 days for chronic carriers 3

Alternative Treatments for Penicillin-Allergic Patients

  • For patients with non-anaphylactic penicillin allergy, first-generation cephalosporins (e.g., cephalexin) for 10 days are recommended 1
  • For patients with immediate hypersensitivity to penicillin, clindamycin or macrolides (erythromycin, clarithromycin) for 10 days, or azithromycin for 5 days are recommended 1

Potential Pitfalls and Considerations

  • Using Augmentin as first-line therapy increases the risk of promoting antimicrobial resistance 1
  • Using Augmentin as first-line therapy increases the risk of higher incidence of gastrointestinal side effects 4
  • The failure of penicillin to eradicate GAS in some cases may be due to reinfection rather than treatment failure 2

Treatment Algorithm for Strep Throat

  • First-line therapy: Penicillin V (500 mg twice or three times daily for 10 days) or amoxicillin (50 mg/kg/day once daily, maximum 1000 mg, for 10 days) 1, 4
  • For penicillin-allergic patients, non-anaphylactic allergy: First-generation cephalosporins for 10 days 1
  • For penicillin-allergic patients, immediate hypersensitivity: Clindamycin or macrolides for 10 days, or azithromycin for 5 days 1
  • Proper diagnosis through laboratory confirmation with either throat culture or rapid antigen detection test is essential before initiating antibiotic therapy 5
