Migraine Treatment Guidelines

Introduction to Migraine Treatment

  • The American Academy of Family Physicians recommends rizatriptan 10 mg for acute moderate-to-severe migraine attacks, providing pain relief in up to 77% of patients within 2 hours, but it must be limited to no more than 2 days per week to prevent medication-overuse headache, and is contraindicated in patients with cardiovascular disease 1

Rizatriptan Dosing and Administration

  • The American Academy of Family Physicians suggests taking 5-10 mg of rizatriptan orally at migraine onset for maximum effectiveness, with a maximum daily dose of 30 mg per 24 hours, and a critical frequency limitation of no more than 2 days per week to prevent medication-overuse headache 2, 1

Absolute Cardiovascular Contraindications

  • The American Heart Association indicates that rizatriptan is contraindicated in patients with coronary artery disease, ischemic vascular conditions, uncontrolled hypertension, history of stroke or transient ischemic attack, peripheral vascular disease, and hemiplegic or basilar migraine 1, 2, 3

Alternative Migraine Treatments

  • The American Academy of Neurology recommends trying a different triptan if rizatriptan fails after 2-3 migraine episodes, as failure of one does not predict failure of others, and suggests eletriptan 40 mg, zolmitriptan 2.5-5 mg, naratriptan 1.0-2.5 mg, and sumatriptan 50-100 mg as alternative oral triptans 2
  • The American Headache Society suggests subcutaneous sumatriptan 6 mg for severe attacks, providing pain relief in 70-82% of patients within 15 minutes, and intranasal sumatriptan 5-20 mg for patients with significant nausea or vomiting 2

First-Line Alternatives for Patients with Cardiovascular Contraindications

  • The American College of Cardiology recommends ubrogepant or rimegepant as primary alternatives when triptans are contraindicated due to cardiovascular disease, as they have no vasoconstriction, and must still be limited to no more than 2 days per week to prevent medication-overuse headache 1, 3

Non-Triptan Acute Treatment Options

  • The American Academy of Family Physicians suggests naproxen sodium 500-825 mg, ibuprofen 400-800 mg, and aspirin 1000 mg for mild-to-moderate migraine attacks, and combination therapy with aspirin, acetaminophen, and caffeine for moderate attacks 2, 3
  • The American Headache Society recommends metoclopramide 10 mg IV or oral, and prochlorperazine 10 mg IV or 25 mg oral for antiemetic and analgesic effects, but with caution due to potential side effects 2, 3

Optimal Combination Therapy Strategies

  • The American Academy of Neurology suggests triptan + NSAID combination as superior to either agent alone for moderate-to-severe attacks, and recommends rizatriptan 10 mg + naproxen sodium 500 mg, and sumatriptan 50-100 mg + naproxen sodium 500 mg 3

Medications to Absolutely Avoid

  • The American Academy of Family Physicians advises against using opioids or butalbital-containing compounds for migraine treatment due to questionable efficacy, dependency, and rebound headaches 3

Prevention of Medication-Overuse Headache

  • The American Headache Society recommends limiting all acute migraine medications to no more than 2 days per week to prevent medication-overuse headache, and initiating preventive therapy if patients require acute treatment more than twice weekly 1, 3

Common Clinical Pitfalls to Avoid

  • The American Academy of Neurology warns against abandoning triptan therapy after a single failed attempt, increasing frequency of acute medication use in response to treatment failure, using triptans and ergot medications within 24 hours of each other, prescribing triptans without screening for cardiovascular risk factors, and using metoclopramide or prochlorperazine chronically 2, 3, 1
