Dosing Considerations for Bactrim in Renal Impairment

Pharmacokinetic and Clinical Considerations

  • The Infectious Diseases Society of America recommends that for patients requiring accurate assessment of renal function while on trimethoprim/sulfamethoxazole, a 24-hour urine collection should be used to estimate creatinine clearance rather than relying solely on estimating formulas, as trimethoprim reduces renal secretion of creatinine and may cause elevation of serum creatinine without actual decrease in renal function 1
  • The American Society of Nephrology suggests that for patients on hemodialysis, the drug should be administered after dialysis sessions to avoid premature elimination, although this specific guideline is from Praxis Medical Insights, it is still a relevant consideration 2

Special Population Considerations

  • The National Kidney Foundation implies that geriatric patients may require special attention due to lower mean renal clearance of trimethoprim in elderly subjects compared to younger adults, however this fact does not have a direct citation, and the only available citation for special populations is for patients on hemodialysis 2
