Second-Line Treatment for C. difficile Infection After Vancomycin Failure
Treatment Options for Recurrent C. difficile Infection
- The Infectious Diseases Society of America recommends fidaxomicin 200 mg twice daily for 10 days as the preferred treatment for first recurrence of C. difficile infection after vancomycin failure 1, 2
- For alternative treatment, vancomycin in a tapered and pulsed regimen, such as 125 mg 4 times daily for 10-14 days, then twice daily for 7 days, then once daily for 7 days, then every 2-3 days for 2-8 weeks, is recommended 1, 2
- Fecal microbiota transplantation (FMT) is highly effective for patients with multiple CDI recurrences who have failed appropriate antibiotic treatments, and is recommended after at least 2 recurrences 1, 2, 3, 4
Adjunctive Treatments
- Bezlotoxumab, a human monoclonal antibody, reduces recurrent CDI by blocking C. difficile toxin B, and is particularly beneficial for patients with CDI due to the 027 epidemic strain, immunocompromised status, or severe CDI presentation 1, 3
Important Considerations
- The World Journal of Emergency Surgery recommends avoiding metronidazole for recurrent CDI due to lower sustained response rates compared to vancomycin and risk of cumulative neurotoxicity with long-term use 5
- Supportive care, including intravenous fluid resuscitation, electrolyte replacement, and albumin supplementation, is essential for patients with severe CDI 5, 6
Special Situations
- For fulminant CDI with ileus, adding intravenous metronidazole to oral or rectal vancomycin may be considered 2, 7
- In patients not responding to standard therapy, intravenously administered tigecycline or passive immunotherapy with intravenous immunoglobulins may be considered 7
Treatment for C. difficile Colitis After Vancomycin Failure
Special Situations: Severe or Fulminant Disease
- For patients with severe disease manifestations, such as hypotension, shock, ileus, or toxic megacolon, despite vancomycin failure, the World Journal of Emergency Surgery recommends increasing vancomycin dose to 500 mg orally four times daily plus adding intravenous metronidazole 500 mg three times daily 8
- In patients with ileus or oral medication cannot reach the colon, the World Journal of Emergency Surgery suggests adding rectal vancomycin 500 mg in 100 mL normal saline every 6 hours as retention enema 8
- The World Journal of Emergency Surgery also recommends considering intracolonic vancomycin via nasogastric tube or trans-stoma administration in patients with ileostomy, colostomy, or Hartmann resection 8
- Early surgical consultation is crucial, but the specific details are not cited with a valid reference
Critical Supportive Measures
- The JAMA recommends discontinuing the inciting antibiotic immediately if still being administered, as failure to stop offending antibiotics is associated with CDI recurrence 9
