Distinguishing Features of Sacroiliac Joint Dysfunction

Inflammatory Back Pain Characteristics

  • The American College of Radiology recommends considering inflammatory back pain in patients with morning stiffness lasting >30 minutes that improves with exercise, particularly in those with onset before age 45 years and chronic duration (>3 months) 1, 3, 4
  • Patients with inflammatory back pain typically experience pain that worsens with rest and improves with physical activity, as well as night pain, particularly in the second half of the night 1, 2
  • Alternating buttock pain is also a characteristic feature of inflammatory back pain, according to the American College of Radiology and Annals of the Rheumatic Diseases 1, 4

Diagnostic Evaluation

  • The American College of Radiology and Annals of the Rheumatic Diseases recommend ordering MRI of the sacroiliac joints (coronal/oblique views with T1 and STIR sequences) to evaluate for bone marrow edema and inflammatory changes in patients with suspected inflammatory disease 1, 2
  • Checking HLA-B27 status is also recommended, although it may be less often positive in IBD-associated cases, according to Gut 3
  • Referral to rheumatology for evaluation of axial spondyloarthropathy is necessary, as delayed diagnosis can lead to missed opportunity for disease-modifying biologic therapy and progression to irreversible disability 3, 4, 5

Critical Pitfalls to Avoid

  • The American College of Radiology and Gut emphasize that treating inflammatory SI joint involvement (axial SpA) as mechanical SI joint dysfunction is a critical diagnostic error, leading to delayed diagnosis and missed opportunity for disease-modifying biologic therapy 1, 3, 5
  • The presence of stiffness that improves with movement is a red flag for inflammatory disease requiring urgent rheumatology referral and MRI evaluation, according to Annals of the Rheumatic Diseases and Gut 2, 3
