Immunological Detection of SSPE

Diagnostic Approach

  • The gold standard for SSPE diagnosis is demonstrating intrathecal synthesis of measles-specific IgG antibodies by calculating the CSF/serum measles antibody index (CSQrel), with values ≥1.5 confirming local CNS antibody production, as recommended by the American Academy of Neurology 1, 2
  • Simultaneous serum and CSF samples are required for measles-specific IgG measurement to calculate the CSQrel, with a sensitivity and specificity of 100% and 93.3%, respectively, when combined with other diagnostic markers 1, 3

Immunologic Signature

  • SSPE exhibits a pathognomonic immunologic feature: persistent measles-specific IgM antibodies in both serum and CSF, which is highly abnormal since IgM typically disappears 30-60 days after acute measles infection, according to the Centers for Disease Control and Prevention 1
  • The persistent IgM distinguishes SSPE from acute measles infection, where IgM becomes undetectable within 30-60 days after rash onset, as stated by the World Health Organization 1, 3

Diagnostic Assays

  • Oligoclonal bands in CSF can be detected and immunoblotted against measles virus proteins, as described by the National Institute of Neurological Disorders and Stroke 5, 3
  • Cell-based assays, such as those using mammalian cells transfected with specific antigens, can be used for neuronal surface antibody detection, although this is more relevant for other autoimmune encephalitides, according to the European Academy of Neurology 4

Differential Diagnosis

  • SSPE must be distinguished from the MRZ reaction seen in multiple sclerosis, which shows intrathecal synthesis against at least two of three viral agents (measles, rubella, zoster), whereas SSPE demonstrates an isolated, extremely strong measles-only response, as recommended by the American College of Rheumatology 1, 3
  • The isolated and extremely high measles antibody titers with elevated CSF/serum index are distinctive for SSPE, according to the International Society for Inflammatory Bowel Diseases 1

Clinical Context

  • SSPE develops from persistent mutant measles virus infection in the CNS, occurring years (typically 2-10 years, but can be as short as 4 months) after the initial measles infection, as described by the World Health Organization 1, 3
  • The persistent antibody response reflects ongoing immune stimulation from CNS viral replication, where the virus establishes true persistent infection in neurons, spreading trans-synaptically, according to the National Institute of Allergy and Infectious Diseases 1
