Linzess Side Effects and Management

Primary Side Effect: Diarrhea

  • Diarrhea is the most common and clinically significant side effect of Linzess, occurring in approximately 16% of patients compared to 2% with placebo, and leading to treatment discontinuation in 3-4% of cases 1
  • The incidence of diarrhea is dose-dependent and varies by indication: IBS-C (290 mcg dose) has a diarrhea rate of 16.3% vs 2.3% placebo 1
  • No serious adverse events related to diarrhea (such as severe dehydration or electrolyte disturbances) were reported in clinical trials, and no deaths attributed to diarrhea occurred 1
  • The American Gastroenterological Association recommends considering the risk of diarrhea when prescribing Linzess, particularly in patients with a history of gastrointestinal disorders 1

Comparative Safety Profile

  • Linaclotide causes significantly more diarrhea than other secretagogues: Linaclotide has a 16.3% diarrhea rate, compared to Plecanatide's 4.3% diarrhea rate 1, 4, 5
  • According to the British Society of Gastroenterology, linaclotide is likely the most efficacious secretagogue for IBS-C, but diarrhea remains the limiting factor 7

Quality of Life Considerations

  • Withdrawal rates due to adverse events remain relatively low (3.4%), suggesting most patients find the benefits outweigh the side effects 1
