Amoxicillin Dosing for Infants

Infection-Specific Dosing Considerations

  • For urinary tract infections in infants 8-21 days old, the American Academy of Pediatrics recommends ampicillin IV/IM (150 mg/kg/day divided every 8 hours) PLUS either ceftazidime or gentamicin over oral amoxicillin 1
  • For suspected bacteremia without identified focus in infants 8-21 days old, the American Academy of Pediatrics recommends parenteral therapy with ampicillin plus ceftazidime or gentamicin rather than oral amoxicillin 1
  • For serious infections in infants under 3 months, parenteral antibiotics are generally preferred over oral amoxicillin, as recommended by the American Academy of Pediatrics 1
  • If higher antibiotic coverage is needed for infants under 3 months, the American Academy of Pediatrics recommends switching to parenteral therapy rather than increasing oral amoxicillin dose 1
