Management of Abnormal Placental Thickness

Initial Assessment and Surveillance

  • The American College of Obstetricians and Gynecologists recommends initiating serial growth ultrasounds every 2-4 weeks to monitor for development of fetal growth restriction in cases of thin placenta, with umbilical artery Doppler studies performed at each surveillance visit 1, 2
  • For thick placenta, detailed fetal echocardiography should be performed to evaluate for cardiac dysfunction and structural heart disease, with consideration of serial ultrasounds to monitor for hydrops development 3

Doppler Assessment and Integration

  • The American College of Obstetricians and Gynecologists suggests performing weekly umbilical artery Doppler if decreased end-diastolic velocity or severe fetal growth restriction is present, with increased Doppler frequency to 2-3 times per week for absent end-diastolic velocity 4
  • Middle cerebral artery Doppler should be added in late-onset fetal growth restriction to influence surveillance and delivery timing 1, 2

Timing of Delivery

  • Delivery timing should be guided by the presence and severity of associated complications, with consideration of delivering at 38-39 weeks for fetal growth restriction with normal Doppler and estimated fetal weight 3rd-10th percentile 4
  • Administering corticosteroids is recommended if delivery is anticipated before 34 weeks, with consideration of magnesium sulfate for neuroprotection if delivery is expected before 32 weeks 1, 2

Frequency of Monitoring

  • Serial growth scans every 2-4 weeks are recommended after diagnosis of abnormal placental thickness, with the specific interval depending on severity of associated findings 1, 2
  • More frequent assessment (2-3 times weekly) is recommended when severe Doppler abnormalities develop 4
