Echocardiography in Heart Failure Diagnosis and Management

Clinical Presentation and Diagnosis

  • The European Society of Cardiology defines decompensated heart failure as deterioration of chronic stable HF that may happen suddenly or slowly, often leading to hospital admission 3, 4
  • Sudden onset palpitations in a patient with known AF history suggests either AF recurrence or another arrhythmia triggering decompensation 3, 4
  • Lethargy and reduced effort tolerance are classic manifestations of low cardiac output and congestion 3, 4
  • Liver congestion is a well-recognized consequence of right-sided heart failure and venous congestion 6, 3, 4

Echocardiography in Heart Failure

  • The American College of Cardiology recommends transthoracic echocardiography (TTE) as the preferred initial imaging test for confirming decompensated heart failure and guiding therapy 1, 2
  • TTE allows rapid identification of acute heart failure with 90% overall accuracy (95% CI, 84%-95%) 7, 2
  • The echocardiogram will definitively answer several crucial questions, including current ejection fraction classification, ventricular wall motion abnormalities, valve function, and left atrial enlargement 3, 4, 7, 2

Guideline-Directed Medical Therapy

  • The European Society of Cardiology guidelines recommend initiating guideline-directed medical therapy (ACEI/ARB/ARNI, beta-blocker, MRA) for HFrEF (EF ≤40%) 3, 4
  • For HFpEF (EF ≥50%), focus on decongestion, blood pressure control, and treating comorbidities 3, 4

Risk Stratification and Prognosis

  • Elevated filling pressures (E/e' >14) require aggressive diuresis 7
  • LV global longitudinal strain predicts need for HF hospitalization 7
  • Doppler-determined transmitral to mitral annular early diastolic velocity ratio (E/e') reflects LV filling pressure and predicts 1-year all-cause mortality 7

Diagnostic Algorithm

  • Order the transthoracic echocardiogram immediately to confirm or refute the clinical diagnosis of decompensated HF, classify the type of HF, identify structural abnormalities, and provide prognostic information 1, 2, 5
  • ECG documentation is necessary to confirm current rhythm 10
  • Natriuretic peptides (BNP or NT-proBNP) are used for diagnostic confirmation 3, 4, 8

Atrial Fibrillation Considerations

  • AF occurs in 20-25% of HCM patients and is linked to left atrial enlargement 9
  • History of AF is independently associated with heart failure-related death, stroke, and long-term disease progression with heart failure symptoms 9
  • Paroxysmal AF episodes can cause acute clinical deterioration with syncope or heart failure resulting from reduced diastolic filling and cardiac output 9
