Diagnosis and Management of Suspected Melanoma on the Sole

Introduction to Melanoma Biopsy

  • The British Journal of Cancer and British Journal of Dermatology recommend performing a complete excisional biopsy with a 2-5 mm margin of normal skin and including subcutaneous fat for suspected melanoma lesions on the sole of the foot, instead of a punch biopsy, to ensure accurate pathological staging and prevent misdiagnosis 1, 2, 3

Biopsy Techniques and Considerations

  • Punch biopsies are not recommended for melanoma diagnosis because they prevent accurate pathological staging and risk misdiagnosis from partial sampling, according to the British Journal of Cancer and British Journal of Dermatology 1, 2, 3
  • The inability to assess maximum tumor thickness with punch biopsies makes it impossible to determine appropriate excision margins, as stated by the British Journal of Dermatology 3
  • Partial sampling risks missing the thickest portion of the lesion, leading to understaging and inadequate initial treatment, as reported by the British Journal of Cancer 1, 2

Excisional Biopsy Procedure

  • The British Journal of Dermatology recommends performing a full-thickness elliptical excision that includes the entire lesion with a 2-5 mm clinical margin of normal skin laterally and a cuff of subcutaneous fat deep to the lesion 3
  • The excision should be oriented parallel to skin tension lines when possible to facilitate re-excision if needed, as suggested by the British Journal of Cancer 1, 2
  • A scalpel should be used to avoid tissue destruction that compromises histological assessment, according to the British Journal of Cancer 1, 2

Pathology Requirements

  • The specimen should be sent with complete clinical information, including patient age, sex, exact anatomic site, and clinical suspicion for melanoma, as recommended by the British Journal of Cancer 1, 2
  • The pathologist must report Breslow thickness, presence or absence of ulceration, Clark level of invasion, mitotic rate, regression, and margin status, as stated by the British Journal of Dermatology and Praxis Medical Insights 3, 4
