Sodium Bicarbonate Drip Duration Guidelines

Monitoring Parameters

  • Serum bicarbonate should be measured at least monthly in maintenance dialysis patients, but more frequently (every 2-4 hours) during active bicarbonate infusion therapy, as recommended by the American Journal of Kidney Diseases and Diabetes Care 1, 2
  • For patients with DKA, venous pH and anion gap can be followed to monitor resolution of acidosis, according to Diabetes Care 2

Duration of Therapy Based on Clinical Scenarios

  • The American Journal of Kidney Diseases and the Journal of the American Society of Nephrology recommend continuing sodium bicarbonate drip until serum bicarbonate reaches ≥22 mmol/L in patients with metabolic acidosis 1, 3
  • In cases of severe acidosis (pH <7.0), Diabetes Care suggests continuing infusion until pH rises above 7.0, then reassessing the need for further therapy 2
  • For cardiac arrest, the BMJ recommends short-term use of sodium bicarbonate during resuscitation efforts, discontinuing once spontaneous circulation is restored and acidosis is corrected 4

Monitoring for Complications

  • Praxis Medical Insights recommends monitoring for metabolic alkalosis (pH >7.45), which can occur with overcorrection 5
