Equivalent Dose of Glimepiride for Glyburide

Dose Equivalence Between Sulfonylureas

  • 6 mg of glyburide is equivalent to 2-3 mg of glimepiride, as recommended by the American College of Clinical Endocrinologists 1

Dosing Considerations

  • The maximum approved daily dose of glyburide is 20 mg daily (regular) or 12 mg daily (micronized), and glimepiride is 8 mg daily, according to the American Diabetes Association 2, 3

Special Populations

  • In patients with renal impairment, glyburide should be avoided if eGFR <50 mL/min/1.73 m², and glimepiride should be started at a lower dose (1 mg daily), as suggested by the National Kidney Foundation 1, 4
  • Acitretin might interact with the glucose-lowering effect of glyburide and should be used with caution, as warned by the American Academy of Dermatology 5

Common Pitfalls When Switching Between Sulfonylureas

  • Failing to consider patient-specific factors such as renal function, age, and risk of hypoglycemia can lead to inadequate glycemic control or increased hypoglycemia risk, as emphasized by the Endocrine Society 1, 4
