Fluconazole Dosing Guidelines for Fungal Infections

Introduction to Treatment Recommendations

  • For moderate to severe oropharyngeal candidiasis, the Infectious Diseases Society of America recommends fluconazole 100-200 mg daily for 7-14 days 1
  • Topical agents are preferred first-line for mild oropharyngeal candidiasis, according to the Infectious Diseases Society of America 1

Esophageal and Systemic Candidiasis Treatment

  • The Infectious Diseases Society of America recommends fluconazole 200-400 mg daily for 14-21 days for esophageal candidiasis 1, 2
  • For candidemia and disseminated candidiasis, the Infectious Diseases Society of America recommends fluconazole 400 mg (6 mg/kg) daily 3

Urinary Tract Infections and Candidiasis

  • For symptomatic Candida cystitis, the Infectious Diseases Society of America recommends fluconazole 200 mg (3 mg/kg) daily for 2 weeks 3
  • For Candida pyelonephritis, increase to 200-400 mg (3-6 mg/kg) daily for 2 weeks, as recommended by the Infectious Diseases Society of America 1, 3

Critical Considerations for Treatment

  • Fluconazole has no activity against Candida krusei and variable activity against C. glabrata, according to the Infectious Diseases Society of America 1
  • Candida isolated from respiratory secretions usually represents colonization, not infection, and rarely requires treatment, as noted by the Journal of Microbiology, Immunology and Infection and Praxis Medical Insights 2, 3

Fluconazole Dosing for Fungal Infections

Candidemia and Invasive Candidiasis

  • The Infectious Diseases Society of America recommends a loading dose of 800 mg (12 mg/kg) on day 1, followed by 400 mg (6 mg/kg) daily, for the treatment of invasive candidiasis, including candidemia, continuing for 2 weeks after documented clearance of Candida from the bloodstream and resolution of symptoms 4, 5
  • For less critically ill patients without recent azole exposure, fluconazole is an appropriate treatment option 4
  • Echinocandins are preferred for critically ill patients, those with recent azole exposure, or suspected C. glabrata/C. krusei infections 4, 5

Urinary Tract Candidiasis

  • For symptomatic cystitis, the recommended dose is 200 mg (3 mg/kg) daily for 14 days 6, 7
  • For pyelonephritis, the recommended dose is 200-400 mg (3-6 mg/kg) daily for 14 days 6, 7

CNS Candidiasis

  • For patients unable to tolerate amphotericin B, the recommended dose is 400-800 mg (6-12 mg/kg) daily 6, 7

Chronic Disseminated Candidiasis

  • The recommended dose is 400 mg (6 mg/kg) daily, continued until lesions resolve on imaging, typically several months 8, 9

Empirical Therapy in ICU Patients

  • For suspected invasive candidiasis in nonneutropenic ICU patients, a loading dose of 800 mg (12 mg/kg) followed by 400 mg (6 mg/kg) daily is recommended, but echinocandins are preferred for empirical therapy in most ICU patients 5
