Diagnostic Approach for Suspected Anaplastic Thyroid Cancer

Initial Diagnostic Steps

  • For a patient with suspected anaplastic thyroid cancer presenting with dysphagia, the National Comprehensive Cancer Network (NCCN) recommends proceeding directly to neck CT scan followed by tissue diagnosis via FNAC or core biopsy, as CT accurately determines the extent of tumor invasion into critical structures, which is essential for determining resectability and treatment planning 1, 2
  • The NCCN guidelines emphasize that CT helps identify the optimal biopsy site, particularly important since anaplastic thyroid cancer can have mixed morphologies and may be difficult to distinguish from other malignancies 1, 3

Diagnostic Imaging

  • CT scan of the neck is superior to ultrasound in evaluating deep structures, vascular invasion, and retrosternal extension, and is critical for assessing the extent of disease and guiding biopsy approach 1, 2
  • Ultrasound is inferior to CT for evaluating tracheal compression, esophageal invasion, or mediastinal involvement, all of which are critical factors in anaplastic thyroid cancer 1

Tissue Diagnosis

  • FNAC or core biopsy should be guided by CT findings to establish histologic diagnosis, with core biopsy preferred if FNA is non-diagnostic 1, 3
  • Pathology review by an expert thyroid pathologist is essential for accurate diagnosis 1, 3

Staging and Treatment

  • PET/CT should be used for staging after diagnosis is confirmed to identify distant metastases, which are present in 15-50% of cases at diagnosis 1, 2
  • Complete resection is one of the few factors that can improve outcomes in anaplastic thyroid cancer, and rapid and accurate staging is paramount given the median survival of only 5 months 1, 2
