Constipation Management Guidelines

Introduction to Constipation Treatment

  • The American Gastroenterological Association-American College of Gastroenterology recommends polyethylene glycol (PEG) as a more effective option for constipation, with an initial dose of 17g daily and no clear maximum dose established, titrating based on symptom response, at a cost of $10-45 monthly, demonstrating a durable response over 6 months 1
  • The American Gastroenterological Association-American College of Gastroenterology also recommends magnesium oxide, with an initial dose of 400-500mg daily and no clear maximum dose established, though clinical trials have used up to 1,500mg daily, with a cost of less than $50 monthly 2

Alternative Treatment Options

  • Stimulant laxatives, such as bisacodyl and senna, are recommended as alternative treatment options, with bisacodyl starting at 5mg daily initially and a maximum of 10mg daily, and senna starting at 8.6-17.2mg daily initially, with a recommended maximum of 4 tablets twice daily 1

Deprescription of Ineffective Treatments

  • The American Gastroenterological Association-American College of Gastroenterology guidelines recommend discontinuing docusate regardless of current dose and replacing it with evidence-based alternatives, such as PEG 17g daily as first-line therapy, magnesium oxide 400-500mg daily, or stimulant laxatives like bisacodyl or senna for more severe cases 1
