Insurance Coverage for Cologuard

Age-Specific Coverage Guidelines

  • The American Cancer Society recommends screening beginning at age 45 with multitarget stool DNA testing as an acceptable option for average-risk adults 2, 3
  • The U.S. Multi-Society Task Force recommends offering colorectal cancer screening starting at age 45, with FIT-fecal DNA (Cologuard) every 3 years as a recommended option for average-risk adults 4, 5
  • The FDA-approved lower age limit for Cologuard use is 45 years for average-risk adults 1
  • The FDA-approved upper age limit for Cologuard use is 85 years for average-risk adults 1

Important Coverage Considerations by Age Range

  • Strong insurance coverage with high certainty of benefit is available for average-risk adults aged 45-75 years, with Cologuard recommended every 3 years 2, 3, 5
  • All major guidelines support screening in the 45-75 age range with multitarget stool DNA testing as an acceptable option for average-risk adults 2, 3, 4
  • For average-risk adults aged 76-85 years, coverage continues but screening decisions should be individualized based on prior screening history, life expectancy, and overall health status 2, 3, 4
  • Never-screened individuals in the 76-85 age range are more likely to benefit and should still be considered for screening 4, 5
  • Life expectancy greater than 10 years is a key determinant for whether screening provides benefit for average-risk adults 6

Critical Caveats

  • Positive Cologuard results require follow-up colonoscopy, which must be factored into the decision to use this test, particularly in elderly patients who may not tolerate colonoscopy 1, 2, 3
  • The test has reduced specificity (87%) compared to FIT (95%), resulting in false-positive rates of 13-40%, meaning many patients will undergo unnecessary colonoscopy 1
