Osteoid Osteoma Treatment Guidelines

Diagnosis and Imaging

  • The American College of Radiology recommends using computed tomography (CT) scans as the optimal imaging modality for patients with suspected osteoid osteoma, due to its high sensitivity for detecting and delineating the nidus, and its essential role in both diagnosis and treatment, with a precision rate of 63% compared to MRI 1

Treatment Options

  • The American College of Radiology suggests that CT-guided radiofrequency ablation is the current gold standard for the treatment of osteoid osteoma, offering a minimally invasive technique with high success rates 1
  • Initial treatment with non-steroidal anti-inflammatory drugs (NSAIDs) or salicylates is a reasonable option for patients with moderate pain and tolerance to these medications, as the tumor may regress spontaneously over 2-6 years 1

Surgical Indications

  • Surgical intervention, such as CT-guided excision or radiofrequency ablation, is indicated for patients with pain resistant to medical treatment, intolerance to long-term NSAID use due to side effects, or unacceptable activity limitation 1

Treatment Algorithms

  • A proposed treatment algorithm includes: confirming the diagnosis with a CT scan, initiating treatment with NSAIDs or salicylates, evaluating clinical response after 4-6 weeks, and considering CT-guided radiofrequency ablation or cryoablation as alternative treatments if symptoms persist or side effects are unacceptable 1
