Vitamin B6 for Pregnancy Nausea

Dosing Regimen

  • The American College of Obstetricians and Gynecologists (ACOG) recommends a dose of 10-25 mg of vitamin B6 orally every 8 hours for treating pregnancy nausea 1, 2
  • Initial dose of 10-25 mg of pyridoxine every 8 hours (three times daily) is recommended for nausea and vomiting of pregnancy (NVP) 1, 2
  • Starting with 10 mg three times daily and titrating up to 25 mg three times daily based on symptom severity and response is a common approach 1, 2

Treatment Algorithm

  • Dietary and lifestyle modifications (small, frequent bland meals, BRAT diet, avoiding triggers) should be started as initial management, followed by vitamin B6 at 10 mg every 8 hours if symptoms persist or are moderate to severe 1, 2
  • Increasing to 25 mg every 8 hours if initial response is inadequate is recommended 1, 2
  • Combination therapy with doxylamine (H1-receptor antagonist) can be added if monotherapy with vitamin B6 is insufficient 1, 2
  • The combination of vitamin B6 and doxylamine is FDA-approved and recommended by ACOG for persistent NVP 1, 2

Monitoring

  • The Motherisk Pregnancy-Unique Quantification of Emesis (PUQE) score can be used to objectively assess severity and treatment response, with scores categorized as mild (≤6), moderate (7-12), or severe (≥13) 1, 2
