Management of Thyroid Storm
Initial Management
- Thyroid storm is an extreme hypermetabolic state associated with high risk of maternal heart failure and mortality if untreated, and initial treatment includes a standard series of medications: antithyroid drugs, iodide solutions, corticosteroids, and supportive care 1, 2
- The underlying cause of thyroid storm should be identified and treated concurrently 2
Acute Phase Treatment
- During the acute phase of thyroid storm, methimazole or propylthiouracil is administered as part of a multi-drug approach 2
- Beta-blockers are essential for controlling sympathetic symptoms until thyroid hormone levels normalize, according to the American College of Cardiology 3, 4
- Monitoring of thyroid function tests helps confirm diagnosis and track response to treatment 2
Transition to Maintenance Therapy
- After stabilization of the acute thyroid storm, patients require ongoing antithyroid medication 1
- The goal of maintenance therapy is to maintain FT4 or FTI in the high-normal range using the lowest possible thioamide dosage, as recommended by the American Family Physician 5, 6
- Monitoring FT4 or FTI every 2-4 weeks is recommended during the initial adjustment period 5
Alternative Treatment Options
- For patients who do not respond to thioamide therapy or develop adverse reactions, thyroidectomy should be considered, and radioactive iodine is another definitive treatment option but is contraindicated during pregnancy, according to the American Family Physician 1
- Dose adjustments should be based on thyroid function tests to maintain euthyroidism 5
