Management of Dermoid Cysts

Diagnosis and Classification

Dermoid cysts are benign congenital tumors classified as choristomas, accounting for approximately 20% of all ovarian tumors, and typically present with characteristic imaging features including fatty and calciferous content, hair, teeth, and cartilage that may be pathognomonic on MRI, as recommended by the American College of Radiology 1, 2
Diagnosis is primarily made through imaging studies (ultrasound, MRI, CT) with ultrasound showing hyperechoic components with acoustic shadowing, hyperechoic lines and dots, fluid-fluid levels, and occasionally floating echogenic spherical structures, according to the Radiological Society of North America 3, 4, 5

For premenopausal patients with cysts <10 cm, optional initial follow-up at 8-12 weeks to confirm diagnosis and annual ultrasound surveillance should be considered if not surgically removed, as suggested by the American College of Obstetricians and Gynecologists 3, 5, 6
For postmenopausal patients with cysts <10 cm and confident diagnosis, annual ultrasound follow-up if not surgically excised and higher vigilance is required due to increased risk of malignant transformation, according to the Society of Gynecologic Oncology 3, 7
Surgical indications for dermoid cysts include cysts >10 cm in size, changing morphology or developing vascular components, symptoms, and risk of malignant transformation, as recommended by the American College of Surgeons 1, 3, 6, 7, 8

Fertility-sparing surgery is recommended for premenopausal women with dermoid cysts, as suggested by the American Society for Reproductive Medicine 1, 2
For postmenopausal women, hysterectomy and bilateral salpingo-oophorectomy may be considered, according to the North American Menopause Society 8

In cases of dermoid cysts with malignant transformation, more aggressive surgical management is required, and squamous cell carcinoma is the most common type of malignancy arising from dermoid cysts, as reported by the National Comprehensive Cancer Network 8
If malignant transformation is confined to the ovary, prognosis is excellent with adnexectomy alone, but for malignant transformation extending beyond the ovary, platinum-based chemotherapy regimens may be considered, though prognosis is poor, according to the European Society for Medical Oncology 8
