Lurasidone Administration and Efficacy
Dosage and Administration
- The American Academy of Child and Adolescent Psychiatry recommends valutare la risposta terapeutica dopo 4-6 settimane di trattamento a dose adeguata 1
- Lurasidone must be taken with food (at least 350 calories), as food increases the AUC by approximately 2 times and the Cmax by approximately 3 times, which is a critical consideration that differentiates lurasidone from other antipsychotics 2
Special Population Considerations
- For patients with moderate renal impairment (creatinine clearance 30-<50 mL/min), the initial dose should be 20 mg/day, with a maximum dose of 80 mg/day 3
- For patients with moderate hepatic impairment (Child-Pugh Score 7-9), the initial dose should be 20 mg/day, with a maximum dose of 80 mg/day 3
Monitoring and Safety
- The British Journal of Psychiatry suggests monitoring patients closely for clinical worsening and the emergence of suicidal thoughts and behaviors, especially in pediatric and young adult patients, and documenting target symptoms, treatment response, and suspected adverse effects 1
- Monitor patients for extrapyramidal side effects, which should be avoided to encourage future adherence 3, 4, 5
Initiating Lurasidone Therapy with Appropriate Dosing and Monitoring
Dosing and Monitoring Recommendations
- Assess therapeutic response after 4-6 weeks of treatment at an adequate dose, as recommended by The Lancet Psychiatry and the Journal of the American Academy of Child and Adolescent Psychiatry 6, 7
- If switching from another antipsychotic, consider gradual cross-titration informed by the half-life and receptor profile of each medication, according to The Lancet Psychiatry 6
