Diagnostic Criteria for Varicose Veins
Ultrasound Documentation Requirements
- The American College of Radiology recommends that duplex ultrasound should be performed to accurately assess reflux, with precise measurement of reflux time in milliseconds (threshold ≥500ms) at the saphenofemoral and saphenopopliteal junctions using standardized techniques 1, 2
- Reflux should be measured at specific anatomical points, including the saphenofemoral junction, saphenopopliteal junction, great saphenous vein at thigh and calf, small saphenous vein, and perforator veins (if clinically relevant) 1, 2, 3
- The technique used to provoke reflux (e.g., pneumatic cuff compression, manual compression, or Valsalva maneuver) must be clearly documented 3
Medical Necessity Criteria
- A vein size of 4.5mm meets the diameter threshold for intervention, as recommended by clinical guidelines 4, 5
- Failed conservative therapy, including compression stockings for 3 months, satisfies the requirement for trial of non-invasive management 5, 6
- Severe symptoms that interfere with daily activities indicate clinical need for treatment, according to clinical guidelines 4, 6
