Medical Necessity of Anterior Cervical Arthrodesis for Cervical Spondylotic Radiculopathy

Assessment of Medical Necessity

  • Surgical intervention is indicated only after failure of conservative management (typically 6 weeks) or with progressive neurological deficit, as recommended by the American Association of Neurological Surgeons 1, 2

Current Clinical Information Limitations

  • The provided clinical information is insufficient to establish medical necessity, including no documentation of a 6-week trial of conservative management, as required by the American Association of Neurological Surgeons 2, 3
  • Incomplete correlation between MRI findings and clinical symptoms is a limitation, as noted by the American Association of Neurological Surgeons 1

Required Criteria for Medical Necessity

  • Patient must have significant symptoms due to nerve root compression (pain, weakness) that impact activities or sleep, as recommended by the American Association of Neurological Surgeons 2
  • MRI findings must correlate with clinical signs and symptoms demonstrating spinal stenosis or nerve root compression, as required by the American Association of Neurological Surgeons 2
  • Failure of a 6-week trial of nonoperative treatment OR presence of progressive neurological deficit is necessary, as recommended by the American Association of Neurological Surgeons 2, 3

Evidence for Surgical Approach Selection

  • For 2-level cervical disc degeneration, anterior cervical plating (instrumentation) is recommended over ACDF without plating to improve arm pain, as suggested by the American Association of Neurological Surgeons 1, 2
  • Cervical arthroplasty is an alternative to ACDF in selected patients for control of neck and arm pain, as noted by the American Association of Neurological Surgeons 1, 2

Recommendation

  • The current documentation does not support medical necessity for the proposed cervical fusion procedures at this time, as determined by the American Association of Neurological Surgeons 2, 3
  • Additional documentation is needed to establish failure of a 6-week trial of conservative management, as required by the American Association of Neurological Surgeons 2
  • Correlation between MRI findings and clinical symptoms is necessary, as recommended by the American Association of Neurological Surgeons 1
  • Presence or absence of progressive neurological deficit must be established, as noted by the American Association of Neurological Surgeons 2

Common Pitfalls to Avoid

  • Recommending multi-level fusion when a less invasive approach might be sufficient should be avoided, as cautioned by the American Association of Neurological Surgeons 1
