Metformin Dosing Guidelines and Renal Safety

Immediate‑Release Metformin

  • Initial dose – Begin therapy with 500 mg or 850 mg taken once daily in adults with normal renal function (Kidney International guideline, 2022) 1.
  • Titration – Increase the dose by 500 mg or 850 mg each day every 7 days until the maximum tolerated dose is reached (Kidney International guideline, 2022) 1.
  • Maximum daily dose – The total daily dose is typically 2000–2550 mg, divided into multiple administrations (Diabetes Care guideline, 2025) 3.

Extended‑Release Metformin

  • Initial dose – Start with 500 mg once daily (Kidney International guideline, 2022) 1.
  • Titration – Raise the dose by 500 mg each day every 7 days, as tolerated (Kidney International guideline, 2022) 1.

Renal Function–Based Dose Adjustments

eGFR (mL/min/1.73 m²) Recommended Action Source
≥ 60 Continue the current metformin dose Kidney International guideline, 2022 [1]
45–59 Consider dose reduction in appropriate clinical contexts Kidney International guideline, 2022 [1]
30–44 Reduce the prescribed dose by 50 % Kidney International guideline, 2022 [1]
< 30 Discontinue metformin and avoid initiation Kidney International guideline, 2022 [1]

Additional Safety Monitoring

  • Gastro‑intestinal tolerance – Gradual dose titration and use of the extended‑release formulation can lessen gastrointestinal adverse effects (Diabetes Care guideline, 2025) 3.
  • Vitamin B12 surveillance – Check vitamin B12 status in patients who have been on metformin therapy for more than four years (Kidney International guideline, 2022) 1.
