Onychomycosis Treatment in Children

Diagnostic Considerations

  • The American Academy of Dermatology recommends obtaining mycological confirmation through potassium hydroxide preparation with microscopy and/or fungal culture before initiating antifungal therapy to avoid treating non-fungal nail dystrophies 1
  • Examination of the child should include checking for concomitant tinea capitis and tinea pedis, as well as checking parents and siblings for onychomycosis and tinea pedis, due to common family transmission 1, 2

First-Line Systemic Treatment Options

  • The American Academy of Pediatrics recommends terbinafine as the preferred first-line treatment for onychomycosis in children, with dosing based on body weight: less than 20 kg, 62.5 mg daily; 20-40 kg, 125 mg daily; more than 40 kg, 250 mg daily 1
  • The treatment duration for terbinafine is 6 weeks for fingernails and 12 weeks for toenails 1, 3
  • The British Association of Dermatologists recommends itraconazole as an alternative first-line treatment, with a pulse dosing regimen of 5 mg/kg per day for 1 week each month, for a duration of 3-5 months depending on severity 1, 2, 3
  • Clinical studies show that terbinafine has an 88% cure rate in children aged 4-17 years, while itraconazole has a 94-100% clinical cure rate in children aged 3-14 years 1, 2, 4

Topical Therapy Considerations

  • The American Academy of Dermatology recommends that topical treatment can be used as adjunctive therapy, with options including amorolfine 5% lacquer and ciclopirox 8% lacquer, but notes that there are no clinical trials demonstrating efficacy as monotherapy in pediatric populations 1, 2, 4, 5

Prevention Strategies

  • The American Academy of Pediatrics recommends prevention strategies including decontaminating or replacing contaminated footwear, applying antifungal powders inside shoes regularly, keeping nails short and clean, and avoiding sharing nail clippers with infected family members 1, 3

Special Considerations

  • The British Association of Dermatologists recommends that if Candida species are identified as the causative organism, itraconazole is preferred over terbinafine due to broader antimicrobial coverage 2, 4
