Referral Pathway for ASD Evaluation in Children

Primary Referral Options

  • The American Academy of Child and Adolescent Psychiatry recommends referring immediately to a multidisciplinary diagnostic team that includes a psychologist, developmental pediatrician or child neurologist, and speech/language pathologist for comprehensive ASD evaluation in a 5-year-old child 1, 2
  • The preferred referral option is a multidisciplinary autism diagnostic clinic, which has the complete team and standardized diagnostic tools required for definitive diagnosis 1, 2

Critical Team Members Required

  • A psychologist is required to conduct cognitive assessment, measure sustained attention, working memory, and processing speed 2
  • A speech/language pathologist is necessary to evaluate receptive and expressive language, pragmatic language skills 2
  • A physician (developmental pediatrician, neurologist, or pediatric psychiatrist) is needed to conduct medical assessment, rule out other conditions, and manage comorbidities 1, 2

What the Evaluation Will Include

  • The comprehensive assessment will involve standardized diagnostic tools, including Autism Diagnostic Observation Schedule (ADOS) and Autism Diagnostic Interview-Revised (ADI-R) 1, 2
  • Cognitive testing, including global ability assessment with verbal and nonverbal components, will be conducted 2
  • Language evaluation, including receptive, expressive, and pragmatic language testing, will be performed 2
  • Adaptive functioning, including assessment of real-world skills across multiple domains, will be evaluated 2
  • Motor skills, including fine and gross motor evaluation, will be assessed, as motor dysfunction occurs in nearly all ASD cases 2
  • A medical workup, including physical examination, hearing screen, Wood's lamp examination for tuberous sclerosis, and genetic testing, will be conducted 1

Timing Considerations

  • The American Academy of Pediatrics recommends that referral for ASD evaluation should not be delayed, as wait times for team-based evaluations can exceed one year 3, 4

Concurrent Actions While Awaiting Evaluation

  • The American Academy of Pediatrics recommends referring immediately to early intervention services or school-based special education, without waiting for formal diagnosis 5
  • Approximately 75% of children with ASD have comorbid psychiatric conditions, including ADHD, anxiety disorders, and mood disorders, which should be screened for 6

Common Pitfalls to Avoid

  • The American Academy of Child and Adolescent Psychiatry recommends that ASD diagnosis requires multidisciplinary input, and single-provider evaluation is insufficient 1, 2
  • Screening tools alone, such as M-CHAT, are not diagnostic and should not be relied upon for older children, who require comprehensive clinical evaluation with ADOS and ADI-R 1, 7

Comprehensive Evaluation and Intervention for Suspected Autism in Children

Importance of Early Intervention and Multidisciplinary Evaluation

  • Behavioral interventions based on applied behavior analysis have the highest-quality data supporting their effects on cognitive and language outcomes in children with autism, with some programs requiring up to 40 hours per week, as recommended by the American Academy of Pediatrics 8

Critical Components of Comprehensive Evaluation

  • No unique facts with citations were found in this section, however, the previous section's fact about behavioral interventions is relevant to the comprehensive evaluation process 8
