Levofloxacin Use in Streptococcal Infections

Clinical Recommendations

  • The IDSA strongly recommends against fluoroquinolones for streptococcal pharyngitis, as older fluoroquinolones have limited activity and newer ones are expensive with unnecessarily broad spectrum 1, 2
  • For necrotizing fasciitis caused by Group A Streptococcus, the combination of clindamycin plus penicillin is the recommended treatment, and fluoroquinolones are not part of recommended regimens 3, 4
  • The Infectious Diseases Society of America recommends levofloxacin for community-acquired pneumonia in adults with comorbidities or recent antibiotic exposure, but it should be reserved for specific clinical scenarios involving streptococci 5, 6

Resistance and Effectiveness

  • Fluoroquinolone resistance in streptococci is increasing, with S. pneumoniae resistance to ciprofloxacin increasing from 0.3% to 3% in the United States between 1997-1998 and 1999-2000 7
  • Macrolide resistance among Group A Streptococcus is <5% in the US, with even lower clindamycin resistance 3

Treatment Guidelines

  • The American Academy of Pediatrics and other guidelines suggest that levofloxacin should be used for documented multi-drug resistant S. pneumoniae infections, but not as a first-line agent for streptococcal infections due to its broad spectrum and the need to preserve fluoroquinolone effectiveness 5, 6

Levofloxacin Use in Alpha-Hemolytic Streptococcal Infections

Microbiological Activity

  • Levofloxacin achieves MIC values of approximately 1.0 mg/dL for pneumococci, making it more active than ciprofloxacin but less active than newer fluoroquinolones like moxifloxacin or gatifloxacin 8
  • Levofloxacin's broad-spectrum activity includes gram-positive, gram-negative, and atypical pathogens, with specific activity against viridans group streptococci 9

Preferred Agents and Resistance Concerns

  • Clindamycin provides excellent streptococcal coverage and is active against both alpha- and beta-hemolytic streptococci 10
  • Institutions using fluoroquinolone prophylaxis must closely monitor local resistance patterns over time 9

Appropriate Use of Levofloxacin

  • Consider levofloxacin for community-acquired pneumonia in adults with comorbidities or recent antibiotic exposure where S. pneumoniae is suspected 8, 9
  • Levofloxacin prophylaxis reduces bacteremia risk from viridans group streptococci in severe neutropenia in pediatric cancer patients 9
  • Avoid empiric use of levofloxacin without considering local resistance patterns, as understanding your institution's antibiogram is critical before selecting levofloxacin 9
