Pre and Post-Surgical Recommendations for Patients with Pancreatic Cancer Undergoing Whipple Surgery

Evaluation and Preoperative Preparation

  • The American College of Surgeons recommends that patients be referred to high-volume specialized centers, as resection rates are approximately 20% higher and mortality significantly lower in these centers compared to low-volume hospitals 1
  • The National Comprehensive Cancer Network (NCCN) guidelines state that discussion in multidisciplinary teams (MDTB) in expert centers is mandatory to define the recommended treatment strategy 2
  • The International Union for Cancer Control (UICC) recommends that tumors be staged according to the 8th edition TNM system 2
  • The NCCN guidelines recommend that resectability be assessed using anatomical criteria, considering the degree of contact between the tumor and peripancreatic vessels (superior mesenteric vein/portal vein, superior mesenteric artery, celiac trunk, and common hepatic artery) 2
  • The evaluation should include biological and conditional characteristics following the consensus of the International Association of Pancreatology (IAP) 2
  • The patient's nutritional status, performance status (PS), and comorbidities should be considered in the treatment decision 2

Surgical Technique and Intraoperative Considerations

  • The American College of Surgeons recommends that the Whipple procedure be performed with pylorus preservation as the preferred technique when appropriate, and requires aggressive nutritional management both pre and postoperatively to optimize oncological outcomes and quality of life 1
  • The NCCN guidelines recommend that the primary objective is to achieve an R0 resection (no tumor cells within 1 mm of all resection margins) through meticulous perivascular dissection 2, 3
  • The International Study Group of Pancreatic Surgery (ISGPS) recommends identifying seven margins: anterior, posterior, medial/superior mesenteric groove, superior mesenteric artery, pancreatic transection, bile duct, and enteric 2, 3
  • The NCCN guidelines recommend that the tumor clearance should be reported in millimeters for all margins to allow for appropriate prognostic evaluation 3

Postoperative Management

  • The Enhanced Recovery After Surgery (ERAS) guidelines recommend a multimodal approach to stimulate intestinal movement: appropriate use of epidural anesthesia, maintaining fluid balance close to zero, oral laxatives, and chewing gum to accelerate gastrointestinal transit 4
  • The American College of Surgeons recommends that the majority of patients can tolerate normal oral intake shortly after elective pancreatoduodenectomy 4
  • The NCCN guidelines recommend that early oral feeding has been shown to be feasible and safe in patients undergoing Whipple procedures 4
  • The American College of Surgeons recommends that artificial nutrition (enteral or parenteral) should be considered selectively only in patients with prolonged delayed gastric emptying 4

Oncological Outcomes and Follow-up

  • The American College of Surgeons recommends that adjuvant therapy is required for all patients with resected pancreatic adenocarcinoma due to high recurrence rates 1
  • The NCCN guidelines recommend that both gemcitabine-based and 5-FU-based chemotherapy regimens have shown benefit in the adjuvant context 1
  • The American College of Surgeons recommends that the role of adjuvant chemoradiation remains controversial, but may be considered for patients with R1 resections 1
  • The NCCN guidelines recommend that neoadjuvant therapy may be beneficial for patients with borderline resectable disease, potentially reducing tumor stage and improving R0 resection rates 1
