Autism Spectrum Disorder Diagnosis and Characteristics

Clinical Presentation

  • The American Academy of Child and Adolescent Psychiatry notes that poor eye contact is a hallmark feature of autism in preschool children, indicating social relatedness impairments 1, 2
  • Refusal to be held indicates marked impairment in social-emotional reciprocity, a key diagnostic criterion for autistic disorder 1, 3
  • Language regression, such as speaking and then stopping, is a recognized pattern in autism, though less common than delayed onset, according to the American Academy of Child and Adolescent Psychiatry 1
  • Total lack of verbal communication at age 5 represents severe impairment, with the American Academy of Child and Adolescent Psychiatry highlighting the importance of early diagnosis and intervention 1, 2
  • Hand flapping is a classic stereotyped motor mannerism seen in autism, as noted by the American Academy of Pediatrics and the American Academy of Child and Adolescent Psychiatry 1, 4, 5
  • Extreme distress when routine is interrupted indicates marked resistance to change, a common feature in individuals with autism 1, 2

Differential Diagnosis

  • Asperger syndrome is excluded due to the presence of significant language delay, which is not characteristic of Asperger disorder, as stated by the American Academy of Child and Adolescent Psychiatry 6
  • Attention Deficit Hyperactivity Disorder (ADHD) does not cause language regression, poor eye contact, hand flapping, or ritualistic behaviors, and its symptoms must be present before age 12 across multiple settings, involving inattention/hyperactivity 7
  • Mental Retardation/Intellectual Disability is excluded as the primary diagnosis because the ability to write alphabets at age 5 demonstrates cognitive skills inconsistent with global intellectual disability, according to the American Academy of Child and Adolescent Psychiatry 1

Clinical Significance and Prognosis

  • Early diagnosis is critical because intervention effectiveness increases with earlier implementation, as emphasized by the American Academy of Child and Adolescent Psychiatry 1, 2
  • The absence of communicative speech by age 5 is a negative prognostic indicator for ultimate outcome, highlighting the need for timely intervention, as noted by the American Academy of Child and Adolescent Psychiatry 1
