Treatment of Atopic Dermatitis in Elderly Patients with Dementia

  • The American Academy of Dermatology recommends pimecrolimus 1% cream as the first choice for treating small lesions of atopic dermatitis in elderly patients with dementia, applied twice daily to affected areas, combined with regular moisturizers 1, 2, 3

Efficacy and Safety of Pimecrolimus

  • Pimecrolimus has proven efficacy in improving disease severity in adults with mild to moderate atopic dermatitis, with high certainty evidence 1, 2
  • It achieves rapid reduction of pruritus, with 81% of patients treated with pimecrolimus versus 63% with placebo achieving a reduction of ≥1 point on the pruritus scale within 1 week (P<0.001) 2, 4

Considerations for Patients with Dementia

  • The simplicity of application is crucial in patients with dementia, and pimecrolimus offers a straightforward regimen without the need for complex potency adjustments or concerns about sensitive areas 1, 2, 5
  • Pimecrolimus can be used uniformly on all body areas, including the face and neck, without restrictions 1, 2

Alternative Treatments

  • If pimecrolimus is not available, low-potency corticosteroids (hydrocortisone 1%) or medium-potency corticosteroids (fluticasone, mometasone) are acceptable alternatives for small lesions, with the advantage of potentially faster anti-inflammatory response but the critical disadvantage of increased risk of skin atrophy in elderly patients 2, 4, 6
  • Crisaborole 2% and ruxolitinib 1.5% have strong recommendations for mild to moderate atopic dermatitis but are second-line options due to cost and limited availability, with crisaborole having high certainty evidence and a favorable safety profile 2, 6, 9
  • Initial regimen: apply moisturizers without fragrance 2-3 times a day to the entire skin, immediately after a warm 10-15 minute bath, and pimecrolimus 1% cream twice daily to lesions until significant improvement is seen 1, 2, 10
  • Maintenance therapy: after initial control, proactive application of pimecrolimus twice a week to previously affected areas can prevent recurrences 2, 7, 8

Common Pitfalls to Avoid

  • Avoid using potent corticosteroids in elderly patients due to increased susceptibility to skin atrophy, even with short-term use 1, 2
  • Do not neglect moisturizers, as they are fundamental and have a steroid-sparing effect 1, 2
