Melatonin Treatment for Sleep Disorders

Adult Dosing Recommendations

  • The American Academy of Sleep Medicine recommends a dose of 5 mg of melatonin taken between 19:00-21:00 for 28 days for adults with Delayed Sleep-Wake Phase Disorder (DSWPD) 1, 2, 3
  • For adults without depression, melatonin treatment increased total sleep time by 56 minutes and decreased sleep latency by 37.7 minutes 4
  • The American Academy of Sleep Medicine suggests starting with 3 mg of immediate-release melatonin for sleep disorders, with dose titration in 3 mg increments if needed 5

Pediatric Dosing Recommendations

  • For children with DSWPD without comorbidities, a weight-based dose of 0.15 mg/kg taken 1.5-2.0 hours before habitual bedtime is recommended 3, 6
  • For children with DSWPD and psychiatric comorbidities, fast-release melatonin at 3-5 mg is recommended, with a weight-based approach of 3 mg if <40 kg and 5 mg if >40 kg 3, 6, 7

Treatment Duration Considerations

  • For adults with DSWPD, a treatment duration of 28 days has shown efficacy 1, 3
  • For children without comorbidities, as little as 6 nights of treatment has demonstrated effectiveness 3, 6
  • For children with psychiatric comorbidities, a 4-week treatment duration is recommended 6

Safety Profile and Adverse Effects

  • No serious adverse reactions have been documented in relation to melatonin use across age groups 8, 5
  • Caution is advised when prescribing to patients taking warfarin and to patients with epilepsy 8

Special Considerations

  • Lower doses of melatonin may be more effective than higher doses due to potential receptor desensitization with higher doses 5
  • Morning grogginess and "hangover" effects are more commonly reported with higher doses 5
  • No significant differences in pubertal development have been observed in children using melatonin compared to non-users 8

Clinical Algorithm for Melatonin Use

  • For adults with DSWPD, start with 3-5 mg taken between 19:00-21:00 and continue for at least 28 days to evaluate effectiveness 1, 3, 5
  • For children with DSWPD (no comorbidities), calculate dose at 0.15 mg/kg and administer 1.5-2.0 hours before habitual bedtime for at least 6 nights 3, 6
  • For children with DSWPD and psychiatric comorbidities, use 3 mg if <40 kg or 5 mg if >40 kg and administer at 18:00-19:00 for 4 weeks 6

Important Caveats

  • Choose reputable formulations when possible for more reliable dosing 5
  • The evidence for melatonin use in primary insomnia is considered low by some guidelines, and many studies on melatonin are of short duration, limiting conclusions about long-term safety 5

Melatonin Dosage and Administration

Adult Dosing Recommendations

  • Prolonged-release melatonin at 2 mg has shown significant reduction in sleep latency specifically in elderly patients (65-80 years) 9
  • The American Academy of Sleep Medicine suggests against using melatonin for primary insomnia in adults (weak recommendation) 10

Dosing Based on Clinical Condition

  • For primary insomnia, 2 mg doses of melatonin showed minimal efficacy for sleep onset, maintenance, or quality, leading to a weak recommendation against its use by the American Academy of Sleep Medicine 10

Important Clinical Considerations

  • Slow-release melatonin appears less effective than immediate-release formulations for sleep onset 9, 10

Safety Profile

  • Caution is advised when prescribing melatonin to patients taking warfarin and to patients with epilepsy due to case reports of potential interactions 11

Quality Control Considerations

  • United States Pharmacopeial Convention Verified formulations can be considered most reliable for melatonin 11

Melatonin Dosing and Administration Guidelines for Sleep Disorders

Adult and Pediatric Dosing Recommendations

  • For elderly patients (>55 years), prolonged-release melatonin at 2 mg has shown some benefit, though the evidence is considered low quality, according to the American Academy of Sleep Medicine 12
  • For children with DSWPD without comorbidities, a weight-based dose of 0.15 mg/kg taken 1.5-2.0 hours before habitual bedtime is recommended, as suggested by the American Academy of Sleep Medicine 13
  • For children with DSWPD and psychiatric comorbidities (including autism spectrum disorders), fast-release melatonin at 3-5 mg is recommended, as stated by the American Academy of Pediatrics 14 and 15

Treatment Duration and Effectiveness

  • For children without comorbidities, as little as 6 nights of treatment has demonstrated effectiveness, according to the American Academy of Sleep Medicine 13
  • The American Academy of Sleep Medicine suggests against using melatonin for primary insomnia in adults (weak recommendation) due to limited evidence of efficacy 12

Melatonin Dosage and Administration

Adult Dosing

  • The maximum recommended dose is typically 12-15 mg, with doses above 10 mg potentially causing receptor desensitization 16
  • For REM sleep behavior disorder, doses range from 3-12 mg at bedtime 16

Safety Profile

  • Common mild side effects include morning headache, morning sleepiness, and gastrointestinal upset 16
  • Use caution in patients taking warfarin or those with epilepsy due to potential interactions 16

Melatonin Dosage and Efficacy

Evidence-Based Dosing Limits

  • Clinical trials have studied doses ranging from 0.5 mg to 10 mg, with the highest studied dose being 10 mg in blind patients with circadian rhythm disorders, indicating a potential therapeutic range for melatonin dosage 17, 18, 19
