Treatment Duration and Dosing Considerations for Teicoplanin
Duration of Treatment by Infection Type
- The recommended duration of treatment with teicoplanin for uncomplicated skin and soft tissue infections is 5-10 days 1, 2
- The recommended duration of treatment with teicoplanin for complicated inpatient skin and soft tissue infections is 7-14 days 1, 3
- The recommended duration of treatment with teicoplanin for uncomplicated bacteremia is 2 weeks 1, 4
- The recommended duration of treatment with teicoplanin for complicated bacteremia is 4-6 weeks 1, 3
- The recommended duration of treatment with teicoplanin for native valve endocarditis is 4-6 weeks 1, 3
- The recommended duration of treatment with teicoplanin for prosthetic valve endocarditis is 6 weeks 1, 4
- The recommended duration of treatment with teicoplanin for pneumonia is 7-21 days 1, 3
- The recommended duration of treatment with teicoplanin for meningitis is 14 days 1, 4
- The recommended duration of treatment with teicoplanin for brain abscess, subdural empyema, or spinal epidural abscess is 4-6 weeks 1, 5
- The recommended duration of treatment with teicoplanin for osteomyelitis is 4-6 weeks 1
Dosing Considerations
- The initial loading dose of teicoplanin is 6-12 mg/kg/dose IV q12h for three doses, then once daily maintenance 1, 3
- For serious infections such as endocarditis or septic arthritis, a higher loading dose of 12 mg/kg twice daily is recommended 6
- For seriously ill patients, a loading dose of 25-30 mg/kg is recommended 1, 2
- The maintenance dose of teicoplanin for patients with normal renal function is 6-12 mg/kg/dose once daily 1, 3
- The pediatric dosing of teicoplanin is 10 mg/kg IV q12h for three doses, then 6-10 mg/kg once daily 1, 4
Monitoring and Safety Considerations
- Routine monitoring of serum levels is not generally required for standard infections, but is recommended for patients with S. aureus endocarditis or septic arthritis, major burns, intravenous drug users, or rapidly changing renal function 6
- The target trough concentration for teicoplanin in patients with S. aureus endocarditis or septic arthritis is ≥20 mg/L 6
- Transition from parenteral to oral agents should be done cautiously, and is not recommended for patients with complicated bacteremia 1, 3
